Application form from individuals completed and signed by the recipient and/or contact person;

Copy of the applicant's/recipient's ID-book photo-page - stamped and signed OK by his doctor, clinic or social worker; or a separate letter from a doctor or clinic why the recipient needs the item/s applied for;

Motivational letter, preferably hand-written and signed by the applicant;

1x Passport-size photo of the recipient attached to the application, top right corner;

1x Full-body photo of the recipient attached, black and white or colour, any size;

BLIA terms and conditions form, signed and dated at the bottom of the page by the applicant;

「Thank you letter to BLIA」 (preferably handwritten), on delivery of the donated items, for example:
「I/we (your or your organisation's name, surname, title, RSA ID No) would hereby like to thank the Buddha's Light International Association for the …… sponsored. I/we greatly appreciate this donation…..」
